When requesting PTS forms from KAMHA (if you are a player who lives in Kingston), or from another hockey center, please remember you need one every year. Here are the links:

A few other things to remember:
1- You only need to request the form once. Additional requests will not get yours quicker.
2- The registrars for all centers are very busy at this time of year, the individuals WILL get to your request.
3- PTS forms for players who will be trying out for the U18 team for the 2023-2024 season will only be available in August.
