I understand that I must be authorized by the Head Coach/Manager of my team to log this request.

I understand that the following fees will be billed to my team's account:
- Tournaments outside of OMHA - 20$
- Exhibition games outside of OHF - 10$

I understand that the confirmation and the Travel/Event Permit will be sent to my Team's Manager.
